Workshop Activity Waiver

I understand that participation in any educational workshop is associated with inherent risk and that any child could potentially be harmed as a result of participation.

By allowing my child to participate in a CFMG workshop, I release and hold harmless CFMG and / or its volunteers from any and all liability including, but not limited to, liability for injuries or damages sustained during or after the event.
